Please complete and sign this agreement call 281-923-3768 and you will be provided the number to send this agreement via fax.
1. All officers and members have approved Texas Triple Scented Candles fundraiser as our fundraiser and two signatures for our
organization/group/team have been provided. We are ready to start selling Texas Triple Scented Candles and request to receive
all selling materials (brochures, order forms, etc) on or before ___/____/_____.
2. I am 18 years of age and have made a firm commitment to use Texas Triple Scented Candles for our fundraiser. I am no longer
considering another company or planning to receive brochures and order forms from another company.
3. I am ready to schedule a kick-off date for the fundraiser and a date to have all orders turned back in to Texas Triple Scented
Candles, Should my organization decide to change these dates I agree to notify Texas Triple Scented Candles for date
approval. If notification is not made dates may be required to remain the same.
4. When making payment for the fundraisers, I understand that Texas Triple Scented Candles only accepts 1 payment in full in
the form of a cashiers check, money order or cash. I understand the candle sellers are required to collect payment at time of
sale/order any checks accepted are made out to your organization and the group/team/organization will keep the fundraiser
profits minus any shipping charges that may apply and then turn in all orders to Texas Triple Scented Candles. I understand all
orders must be given to Texas Triple Scented Candles at the same time on the same day along with payment for every item
ordered and any shipping charges that may apply. Make payable to: Teri Askew-Texas Triple Scented Candles.
5. Texas Triple Scented Candles will deliver my fundraiser orders for FREE locally within a 20 mile radius of Porter, TX 77365
when you sell a minimum of 30 items, all others shipping charges will apply based on weight. A minimum of $350 in sales per
group/team/organization must be sold to qualify for the fundraiser profits/discounts.
6. Notification of any missing, damages due to shipping, or exchange orders shall be made within 10 days of the date that you
received your order(s).
7. All orders will be delivered or shipped at the same time and within 18 – 29 business days from the date of payment and order
placement.
8. To help keep cost down and continue to bring our fundraising groups quality handmade products and great pricing we need
any unused participant packets returned. Packets include brochure, order form, 9 scent samples and the collection envelop.
The number of unused packets is determined by the actual number of participants that sold fundraiser items. Example when
provided with 10 fundraiser packets and 9 participants made fundraiser sales then only 1 packet would be due to be returned.
By signing below you agree that any participant packets not returned before or during delivery of your candle order a $2.00 fee
per packet will be applied.
